Enables the seamless, secure flow of patient clinical information across settings
Interoperability is at the heart of a successful health information exchange (HIE) enabling the seamless and secure flow of patient medical records between systems from different vendors. This flow benefits patients and caregivers by insuring the right information is provided at the point of care at the moment of need.
CareAlign CareExchange is a proven solution that establishes the foundational infrastructure to facilitate sharing clinical information within a community of healthcare providers. With CareExchange, multiple disparate information systems will be able to share data as a kind of library of medical records, where each system can either contribute to or look at medical records.

CareAlign CareExchange supplies the necessary infrastructure to enable medical record sharing, including:
- Patient identity manager – to track all patients in a community to identify a single patient receiving care across multiple care settings by multiple providers
- Document registry – to keep track of all the medical records and document locations
- Audit repository – to create a log of all transactions for all system each time a new patient medical record is requested; the audit repository is a central place for all systems in the network to store their audit log transactions
How CareExchange works with CareAlign
CareAlign interacts tightly with the CareExchange XDS infrastructure, providing a secure access to the library of documents.
Within CareAlign, practitioners are able to look up their patients as they would normally while CareAlign silently and quickly queries CareExchange for additional clinical care documents for the patient. The documents are then added to the patient’s chart so that providers can seamlessly discover and read documents created by other systems in the community.
When the practitioner is ready to view a document, they simply click on the document link, and CareAlign downloads the document from CareExchange.
CareAlign CareExchange provides the core interoperability for HIE upon which an IDN or community can build the numerous features that make up a fully functional and successful HIE.
