CareAlign® CareCollaborate is based on almost two decades of physician-developed technology at Vanderbilt Medical Center and is now productive use by IDNs and healthcare communities across the nation to form the infrastructure of private and public HIEs.

Features-rich and comprehensive, CareAlign CareCollaborate expands meaningful use of data by incorporating virtually any clinical data source to exchange between EHRs or display the comprehensive, patient-centric data in a longitudinal record that is available to providers across multiple communities from any secure Web connection via our clinical portal.

Through the CareCollaberate portal, providers have access to a powerful array of clinical tools and workflow functionalities, including consolidated lab and diagnostics views, radiology images, status and critical values alerts, flowsheets and provider rounding lists.

Configuration Options
CareCollaborate offers additional certified EHR-Lite tools designed expressly to help providers without full EMR/EHR capabilities meet meaningful use requirements and create greater efficiencies in workflow. Examples of such tools are order and results delivery, ePrescribing and medication reconciliation.

CareCollaborate also provides a patient portal to help patients engage in their own care by providing a patient-friendly platform through which they can view results, communicate with their provider and receive follow-up reminders and health alerts. The portal is also compatible with commercial Patient Health Records (PHRs) that the patient may already be using, such as Dossia or Microsoft’s HealthVault.
